Children and young people who are on the SEND register at SEN Support should have their outcomes reviewed at least three times a year. In an Early Years setting this is any child with a SEND Support and Outcomes plan. The reviews should collaborate with all those involved in supporting the child or young person. The purpose of the review is to:

  • Regularly discuss whether the support is making a difference and if the plan needs to change.
  • Discuss how effective the additional provision has been and the impact on the child or young person.
  • Review assessments before and after the additional provision/intervention
  • Think collaboratively about the next steps with the family.

As part of the review the child or young person’s pupil profile, pupil passport, targets/outcomes, support plan or similar may be updated, and data tracking the impact of any interventions will be shared and updated.
