Updated Oxfordshire transition guidance

Oxfordshire County Council has published updated comprehensive transition guidance to support successful transitions for all Early Years learners, including vulnerable learners and children with special educational needs who may need an enhanced transition, as they move to new settings.

Effective transition management is key for child development and emotional well-being. The forms included in the Early Years transition materials have been developed in collaboration with early years settings and primary schools and should provide all the information required to support transition. 

In Oxfordshire we are encouraging schools and settings to use these materials rather than localised versions to increase consistency of information sharing. The transition framework provides schools and early years settings with a structured suite of materials, including agreed principles for good transitions, clear expectations around information sharing, and practical tools to support collaboration with families and other professionals. 

Resources include transition timelines, screening tools to identify children who may need enhanced transition support, “All About Me” documents, transition summary forms, and enhanced transition action plans to support joint planning. The materials aim to promote consistent, inclusive practice across Oxfordshire and help settings and schools work in partnership to ensure every child experiences a positive and successful transition.
