Transition Resources For Schools

There is a new video to explain the Oxfordshire SEND Transition Guidance. The guidance includes a range of resources to help schools, settings and families plan and support successful transitions. 

The materials include guidance, screening tools, information-sharing documents, transition passports and action plans designed to promote positive transitions for all learners, particularly vulnerable pupils and those with SEND who may benefit from enhanced transition support. 

The resources support transitions from Early Years into Reception, as well as from primary to secondary school.
