Referral pathway

Referral is via an Early Help Assessment; you can also contact the LCSS for an informal discussion.

The current and approved versions of the Oxfordshire Threshold of Needs, Early Help Assessment Form and Team around the Family documentation (formerly TAC and CAF) can always by found on the oxfordshire Safeguarding Children Board  website.

People making a referral into service, or who have a situation they wish to discuss with a worker, can contact the Locality and community Support Service for a dicussion or no names consultation.

Locality and Community Support Service

The Locality and Community Support Service (LCSS) has been created as part of Oxfordshire County Council’s, Children’s Services Integration Programme to support partner agencies across Oxfordshire. It is a professional facing service (including community/voluntary sector). All schools should have an LCSS link worker. If you are no sure whose yours is yet, you can find them on the Oxfordshire Safeguarding Children Board website.

Children & Family Centres

Your local Children & Family Centre combines services previously offered by Children's Social Care, Early Intervention Hubs and Children's Centres, into one joined-up service for children and families, based in eight Children and Family Centres.

Substance Misuse Service

 The Aquarius service offers support to those who misuse substances and also those who are affected by their family’s use for young people aged 11 – 19 (up to 25 for those with complex needs).

YOS

For young people who have come to the end of YOS intervention but require ongoing support, YOS will conduct an Early Help Assessment to ensure continuity of support.

Young people who are eligible to be diverted from the criminal justice system due to being part of vulnerable cohort include children and young people on child protection plans, children who are looked after and any who are open to the Family Support Service.
